Predictors Influencing The Change in Health status of elderly in community care

Primary Objective:    

  • To describe the current health status of clients receiving CACP and EACH (including EACH Dementia) through the BCC service organisation. 

Secondary Objective:  

  • To determine the change in health status of BCC clients over 12 months duration (0, 6mo, 12 mo);
  • To determine the predictors (risk factors) associated with movement between packages of care (CACP, EACH, EACH Dementia);
  • To determine level of carer strain associated with caring for clients on BCC packages of care;
  • To determine the differences in client characteristics based on living in rural and metropolitan settings.

Study Design:  Prospective longitudinal observational study

Duration:  15 months (3 month recruitment with 12 month follow up)

Outcomes: The outcomes of the study will be:

1. A detailed description of the health related status of clients enrolled in

    a)  CACP
    b)  EACH - including EACH (Dementia)

Health related status is defined as the physical, emotional and social wellbeing of subjects enrolled in CACP and EACH as also defined by World Health Organisation

2. The determination of health change over time and the predictors (risk factors) associated with movement between packages of care (CACP, EACH, EACH Dementia).
